Position: Social Worker

Department: Social Work and Patient Flow

Status: Casual

Hours: Monday to Friday

Site: all sites

Responsibilities:

As part of a multi-discipline team, the incumbent will provide clinical social work services to inpatients in medicine, surgery, ICU, Child and Adolescent Psychiatry, Adult Inpatient Mental Health and maternity.

-Be a member of a multidisciplinary team, providing clinical social work services to medicine, surgery, ICU, Child and Adolescent Psychiatry, Adult Inpatient Mental Health, and the maternal care unit.

-Conduct comprehensive social work and psychosocial assessments on inpatients, using a variety of modalities and in accordance with professional standards, clinical policies and practices.

-Alone or in collaboration with other team members, provide selected therapeutic interventions to patients and families, supportive counseling and education, group interventions, case management, resource co-ordination, community outreach and advocacy.

-In collaboration with professional colleagues on the unit, promote high standards of ethical clinical care.

-Participate in team conferences, educational/interdisciplinary rounds, and educational rounds.

-Participate on hospital committees as needed.

-Liaison with Public Guardian and Trustee as needed.

-Active liaison with community partners, including initiatives to improve collaborations, develop new resources

-Other duties as assigned.

Qualifications:

Halton Healthcare is seeking individuals who are competent in their chosen profession; patient/customer focused; team players; effective communicators; caring and respectful and are fiscally responsible.

  • Master of Social Work required
  • Must be a member of the respective college
  • Previous Relevant Experience Required
  •  Experience providing scope of practice social work to inpatients on medical, surgical, ICU, psychiatry (both child and adolescent and adult mental health), and Maternal Child.
  • Previous child welfare experience preferred.
  • CBT and Brief Solution Focused therapy experience preferred.
  • Strong knowledge of community resources and demonstrated ability to work collaboratively and effectively with community partners.
  • Familiarity with Mental Health Act, Consent to Treatment Act, Substitute Decisions Act, and Healthcare Consent Act.
  • Demonstrated ability to maintain an evidence-based practice approach.
  • Demonstrated psychiatric assessment, counseling, and case coordination skills.
  • Demonstrated excellence in interview skills and ability to provide comprehensive and timely psycho-social assessments.
  • Previous experience working with the Public Guardian and Trustee Office.
