The IBD Nurse will be an active participant in the care of the IBD patient and will require exceptional organizational skills and interpersonal skills to effectively deal with conflict and stressful situations. The Nurse will be a patient advocate and work collaboratively with health care providers and patients and their families. The IBD Nurse will have strong critical thinking skills and be able to effectively deal with unexpected changes in clinical situations in a positive manner. The IBD Nurse will continue to broaden their knowledge base and continue to update themselves on changes in clinical practice in the therapeutic area.
General Patient Care Management:
The IBD Nurse helps manage the ongoing coordinated care of patients followed by Specialists that are infused/injected with biologic drugs (for example: Remicade, Stelara CD etc.) at an Inviva Clinic. The IBD Nurse patient caseload is comprised of patients with inflammatory bowel disease requiring biologic treatment. The IBD Nurse is responsible for the multi-disciplinary team coordination of patient care. The IBD Nurse will act as a clinical resource for staff and patient educator and advocate. More specifically, the caseload may include patients in one of the following situations:
Patients with inflammatory bowel disease and identified special care needs. These needs may include:
Disease and medication education
Coordination of referral to appropriate health care professionals
Biologic set up for medications such as Remicade, Stelara CD etc.
TB testing referrals
Home Care referrals
Inflammatory bowel disease patients in the peri-operative phase and in the post-hospital phase:
Facilitate direct admission to hospital and coordinate care of patient under the direction of doctor to ensure patients issues are addressed and to prevent ER visits
Coordinate with inpatients and hospital staff to arrange for outpatient therapy and follow up
Follow up and coordination with IBD patients requiring
Intravenous antibiotic therapy
Under the guidance of the Specialist, the IBD Nurse will help identify the patients specific needs and assist with scheduling procedures, infusions and appropriate imaging and ancillary clinic appointments
Specific Job Duties of the IBD Nurse:
Initiation of Biologic Therapy
Patient education relative of the biologic product(s) and their efficacy and safety.
Reviewing patient’s immunization history and making recommendations to which vaccines are needed under the guidance of the Specialist.
Linking the patient and their family with the appropriate biologic program coordinator, to ensure patient can get enrolled the in the program
Completing all the necessary forms for drug acquisition and insurance.
Ensuring patient has had TB testing completed and results faxed to the program, prior to first biologic infusion/injection.
Ensuring patients have all standard blood work for initiation of biologic drug completed prior to first infusion/injection.
Ensuring patients have had a Chest X-ray within the last 6 months. If they haven’t, ensure that patient is given Chest X-ray requisition and that same is completed prior to first infusion/injection.
Review with the patient the importance of making sure their family Physician performs annual routine breast exams, pap smears, skin examinations for atypical moles and annual dental exams and dental care with their Dentist.
Review with patients what they can expect at their first infusion/injection. Review treatment reaction symptoms with patient and discuss length of appointment, pre medications and clinic guidelines as appropriate.
Review with patient who their Program Coordinator is, their Patient Care Representative and IBD Nurse and who to call with what concerns. Give patient Important Contacts list with all contact information.
Ongoing follow up: The IBD Nurse will continue to assess patients ongoing therapy for efficacy and adverse events. The IBD Nurse will communicate with the doctor regarding any change in status and need for additional therapy or address a need for a change in therapy.
Annual Reviews – ensure that Medical Orders are up to date and accurate and ensure that patient has had annual lab review and appointment with Specialist on an annual basis.
The IBD Nurse will review abnormal lab results and diagnostic imaging results with the Specialist and carry out any changes in care, additional diagnostic and lab testing that are ordered.
The IBD Nurse will administer Vitamin B12, and make arrangements for Vitamin D, Testosterone, Vitamin A and E therapy under the direction and guidance of the Specialist as determined from results of patients’ blood work.
The IBD Nurse will arrange Iron infusion therapy for patients under the direction and guidance of the Specialist.
The IBD Nurse will work independently and be responsible for evaluating all IDB – related medical and nursing problems identified in telephone calls made directly to the IBD Nurse.
The IBD Nurse may respond to these calls either by facilitating appointments, and/ or ordering additional tests under the guidance and direction of the Specialist.
All phone calls,issues discussed and outcome of phone calls will be documented in the patients chart in the Nursing Notes.
The IBD Nurse will follow up on all testing and/or procedures ordered by the Specialist, with the patient, and follow up with the Specialist on findings and collaborate on further treatment plans.
The IBD Nurse may independently manage nursing problems that are a part of a patient’s on-going care. Medical questions will be answered within the limits of the IBD Nurse preparation and practice in the specialty of inflammatory bowel disease, but will ultimately be supervised by the Specialist. Any medical questions that cannot be answered by the IBD Nurse will be forwarded to the Specialist for further direction and or follow up.
Psychosocial Support and Education
The IBD Nurse will provide general support to the patients followed by the Specialist that are receiving biologic therapy at the Inviva Clinic.
The IBD Nurse is also responsible to identify any educational needs of the patients and their families and will utilize appropriate education resources to provide patients and their family’s education.
Third Party Assistance
The IBD Nurse will assist with non-medical patient care such as:
Completing Special Authorization forms
Complete appropriate documentation for release of medications through Pharmaceutical support programs.
Skills and Abilities
The IBD Nurse will have the following experience, skills and abilities:
Lisenced Registered Nurse (BScN) in the province of practice (Alberta)
2 years minimum experience in IBD biologic therapy treatment or similar type role which would demonstrate prerequisite expertise and knowledge
Demonstrated ongoing education in therapeutic area and/or willingness to engage on ongoing education
Demonstrated strong critical thinking skills to make independent decisions where appropriate within nursing scope of practice
Demonstrated strong interpersonal skills to be an active participant in a multidisciplinary team providing holistic patient care
Active access to Alberta NetCare technology (preferred)
